Form E111 and Medical care while travelling within the EU
NOTE: Form E111 has and usage has been revised for 2005. Forms are now downloadable. Be sure to download (bottom of this page) both forms and take to PO for stamping.

If you fall ill or have an accident most of the costs of treatment and medication are recoverable when visiting member countries, but in order to do so it is vital to obtain form E111 and have it validated before travelling. It can be obtained from any post office, where it should also be validated after completion. The good news is that it is only necessary to do this as a one-off (provided you don’t lose it!). However, as you need to hand in part of the form if you need treatment, it is a good idea to take a photocopy, which is perfectly acceptable.

Please note that Form E111 is not a substitute for comprehensive travel insurance and will not cover you for cancellations, lost luggage, personal liability etc.

There is no time limit on the new E111. It remains valid indefinitely so long as you remain ordinarily resident in the UK. You will only need to apply for another E111 if you use it to claim for treatment or if you mislay it. Forms prior to Oct 2004 are now obsolete.

Form E111 covers both you and your dependent children - up to the age of 16, or 19 if they are still in full time education. When your children pass these ages they can get an E111 in there own right and you should apply for another form without the children included..

Complete both form E111 and CM1 and take to Post Office to be stamped before departure.
